3. Issues, risks and concerns
| ID | Type | Description | Status | Owner |
|---|---|---|---|---|
| IR-001 | Risk | Evidence preservation exposure: vulnerable institutional/archive sources may degrade before retrieval. | RED/AMBER | ROLE-PM |
| IR-002 | Issue | Task population discrepancy: obsolete 47-task dashboard superseded by 80 live tasks; reporting baseline must use 80. | AMBER | ROLE-PM |
| IR-003 | Risk | HMLR/ICR timing: reliance on generic correspondence dates risks missing specific deadlines. | AMBER | ROLE-PM |
| IR-004 | Concern | Woodland decision (P2-T) must remain controlled and evidence-led before any irreversible route. | AMBER | ROLE-PGM |
| IR-005 | Risk | Fraud/conduct stream activation without qualifying evidence would undermine programme credibility. | GREEN/CONTROLLED | ROLE-PGM |
| IR-006 | Issue | Unknown professional/external costs remain TBC; no budget variance can be asserted without recorded actuals. | AMBER | ROLE-PGM |
4. Plans moving forward
- Resolve the 6 October and 9 October deadline controls.
- Progress P1 evidence-recovery work and the master evidence-capture protocol.
- Maintain the HMLR/ICR, woodland and external-response controls.
- Complete the four working PIDs and prepare the G0 decision pack.
- Reconcile the risk, issue/change, quality, lessons and decision registers against live execution.
- Address IR-001 to IR-006 through the weekly plan.
- G0 gate remains forecast for 18 December 2026.
